The U.S. Department of Health & Human Services' Administration on Developmental Disabilities would like input from the public on how they can best meet the needs of individuals with developmental disabilities and their families.ADD is asking for input through an online survey to decide where their next focus should be.  The survey can be completed online until September 1, 2010. Care Notebook Information

Our care notebook offered below for you to download (all or part of it), is a valuable tool in achieving a medical home/family centered care. This record keeping system will allow you to have all pertinent information about your child and his/her needs in one place. This book should follow your child to all appointments, tests, therapies, IEPs, etc. This will help in the coordination of care  as they will see all interactions by other professionals involved in your child's care.

The instructions and pages to download as well as what is a medical home and family centered care were explained in trainings across Delaware. If  after looking at the materials below, you have questions please call our office and we will be happy to assist you in putting together this valuable tool. The References section contains information pertaining to immunizations, important phone numbers, and contact information of doctors and other medical personnel. The References section also contains a glossary of important terms that could pertain to your child.
